A & A REPORTS - TO ALL CLUB SECRETARIES - DECEMBER 2009

Time for submittal of the first quarter A&A Reports is approaching. Governor Sandi would love to see 100% of INDIANA SOUTH Clubs participating in the A&A program this year. The first quarter report covers activities from October 1 through December 31. It must be submitted on the internet by 11:59 pm on January 10, 2010

INSTRUCTIONS

If you are reading this on the web click this link directly to the Indiana South District 2009-2010 A &A FORM.

Otherwise, go to www.optimistindiana.org, click on FORMS, then on A &A FORM.

Click drop down menu and select your Club, put your Club number in as your password.

Click "Go To This Selected Club".

Click "Review or Make Changes to your clubs A&A Form"

Review the data in the header concerning your Club. If there's a problem let Kathy Williams know. Her email is kathywilliams1@insightbb.com.

Insert your Club membership total as of December 1, per Optimist International, in the block next to "Dec 31"

From here on, all checks must go in the "Total First Qtr" column ONLY

Go to Para. A Item A.1, default is LESS. If your Dec 1 official membership is equal to or more than Oct 1, click drop down menu and check proper category. Items A.2 and A3. check "Yes" if appropriate

Go to Para. B Item B.1, check 0, 1, or 2 as appropriate. Items B.2 and B.3, check the number of activities in the "Total First Qtr" column, then list the activites and the number of kids served in the large box under "List Activities Below". USE A SEPARATE LINE FOR EACH ACTIVITY. eg: 1. Tri-Star Football, 75 2. Coloring contest, 110 After listing all activities, add the number of youth served and insert in the Q1 box.

B4 through C13, click drop down menu and check appropriate answer.

C14, Add total dollars contributed by Club and individual Club members to OIF and divide by the number of members listed in header for Dec 1. Insert number in box.

C15 through E6, click drop down menu and check appropriate answer.

E7 through F15 do not apply to the 1st quarter.Addition will be done by the computer.

Type your name in the bottom box. Click SAVE. YOU ARE DONE!! THANK YOU.
